The athletes whose names are listed must complete the information form below. At the request of the athletes, a permission letter will be provided to the schools/works, and the relevant sections of the information form must be completed by 16:00 on Wednesday, January 31, 2024. Permission letters won’t be sent to those who don’t give work/school location information on the form. You are sincerely requested to fill in all of the information on the form completely.
